It is with great sadness that we announce the death of Paul W. Cook D. D. S. of Wauwatosa, Wisconsin, born in Viroqua, Wisconsin, who passed away on December 18, 2020, at the age of 63, leaving to mourn family and friends. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.
He was predeceased by: his father Dr. Wayne. He is survived by: his mother Esther Cook (Sisbach); his wife Cynthia Faulkner; his sisters, Barbara Leis (Mike), Susan Schreiner (Dave) and Joan Lind (Carl); and his nieces and nephews, Christopher Leis (Connie), Michelle Feucht (Greg), Adam, Allison, Andrew Schreiner, Maria Clevy (Ian), Breanna Zhang (Eric) and Craig Lind. He is also survived by great nieces and nephews, other relatives and friends.
