He is survived by: his wife Theresa Jane Koshollek; his son Timothy Koshollek (Michele); his grandchildren, Shyla and Jayce Koshollek; his siblings, Robert Koshollek and Joan Frazer; his extended family, Matt and Tesa Bohm; and also, Dawson, Garret and Sawyer.
