He is survived by: his mother Jackie Olson; his wife Angela Dvorak; his children, James McCullick, Cody Dvorak, Tanya Dvorak, McKenzie Lloyd (Dan Thompson), Breanna Lloyd (Jacob Kingsley), Kaitlynn Lloyd and Nathanial Lloyd; his grandchildren, Grayson Bloomingdale, Rylee Kingsley, Scarlett Kingsley and Norah Kingsley; his siblings, Verdan Dvorak, Vincent Dvorak (Therasa), Carl Olson, Keith Olson and Christine Holen; his parents-in-law, Daniel and Debbie Pranke; and his sisters-in-law, Teri Esparza, Alisa Satian (Mike) and Rebecca Oquendo (Angel). He is also survived by other relatives and friends.
