He was predeceased by: his parents, Joe Kooistra and Bertha Kooistra (Advocate); his wife Lillian Dammeir; and his brothers, John, Jack and Joe Jr..
He is survived by: his nieces and nephews, Anita Ryan, Karen Kolls, Kathleen Benoy, John A. Kooistra, Joel Kooistra, Larry Henning and William Henning; and his sister Nellie Henning.
In lieu of flowers memorials may be given to American Cancer Society or Saints Simeon & Anna Anglican Church, 189 Plafield Ct., Walworth, WI 53184.
