Burk (Walworth, Wisconsin), born in Chicago, Illinois, who passed away on November 2, 2018, at the age of 86, leaving to mourn family and friends. She was predeceased by: her parents, Edward Hoffman and Arlene Hoffman (Cook); and her siblings, Harold and Linda. She is survived by: her husband Terrence P. Burk; her children, Douglas Burk (Barbara), David Burk (Kim), Amy Ludwig (David) and Jennifer Cook; her grandchildren, Sarah, Eric, Ryan, Nate, Dan and Rachel; her brothers, Tod Hoffman (Joan) and Raymond Hoffman (Sandy); and her siblings-in-law, Bea Hoffman and Bob Trivers. She is also survived by many nieces and nephews.
