He is survived by: his wife Mary Brooks; his children, Sarah Couch (James), Philip Couch (Donna), Paula Sass (David) and Julie Grooms (Lee); his grandchildren, Alysson, Bryanna, Tommy, Joey, Emi, David and Bobby; and his siblings, Lois Carol Slater (Steve), William David Couch (Donna), Charles Robert Couch (Bonnie), Steven Underwood (Karen), Rebecca Kazberouk and Gerald Underwood. He is also survived by his wife, children and grandchildren.
