He was predeceased by: his parents, Milford Lee and Hazel Lee (Gunderson); and his daughters, Cindy and Tammy. He is survived by: his wife Sandra B. Collins of Tomah; his daughter Kathy of Milwaukee; and his stepsons, Eddie and Brian. He is also survived by other relatives and friends.
