He was predeceased by: his parents, Charles Hornby and Betty Hornby. He is survived by: his wife Debra; his children, Nicole Cissell (Martin), Brian Upesleja and Jessica; and his grandchildren, Jordan, Chase, Charleston and Theodore. He is also survived by other relatives and friends.
