We are sad to announce that on June 30, 2018, at the age of 80, Judith Anna Moniak (Sun Prairie, Wisconsin), born in Edgerton, Wisconsin passed away. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.
She was predeceased by: her parent Maxine. She is survived by: her friend Mary Lindmark Bauer; her children, John Jurgensen (Lynn) of Sun Prairie, Paul Jurgensen of Chicago and Jane Jurgensen (Mark) of Ames; her grandchildren, Sarah Elizabeth, Paul Richard Moniak, Atticus John, Eliot Max and Ian Vail Jurgensen; and also Ellen Munson.
In lieu of flowers, contributions may be directed toward New Perspective Senior Living of Sun Prairie, Wisconsin, AgraceCare Hospice Care of Fitchburg, Wisconsin, The Alzheimer's Association or Planned Parenthood of Illinois.
