He was predeceased by: his parents, Paul Kozlowski and Helen Kozlowski; and his sisters, Lorraine Swanson, Virginia Kerney and Joann Shaurette.
He is survived by: his wife Donna; his children, Barbara Shefchik (Brian), James Kozlowski, Kim Schreiber (David), Mike Kozlowski (Tish), Terry Kozlowski and Pamela Kozlowski; his grandchildren, Adam Shefchik (Amy), Andrew Sherfchik (Brad), Katie Shefchik (Fiance Kyle), Amanda Shefchik, Kayla Shefchik, Jacob Schreiber, Joshua Schreiber, Heather Kozlowski (Liz), Ryan Kozlowski, Krystal LePak (Tyler) and Tyler Kozlowski; his great grandchildren, Haylee, Faith, Evelyn and Caleb; and his siblings, Henry Kozlowski and Beverly Hall.
