He was predeceased by: his parents, Joseph Jakubiak and Helen Jakubiak (Bayer); his wife Mary Wilkinson; and his sons, Jimmy and Johnny.
He is survived by: his children, Laura Konkol (Maynard) of Stevens Point, Terrance Wilkinson of Stevens Point, Thomas Jakubiak Jr. (Marsha) of Amherst Jct and Michael Jakubiak of Stevens Point; and his sister MaryAnn Lucas. He is also survived by 8 Grandchildren, and 3 Great grandchildren.
In lieu of flowers a memorial in his name will be established.
