He was predeceased by: his parents, Tony Butkus and Eve Butkus; his father-in-law Richard Montgomery; his siblings-in-law, Huey Montgomery and Diana Montgomery; and his nephew Anthony Butkus.

He is survived by: his wife Barbara; his children, Johnny, Lori Cataldo (Mike), Cheryl Ligman (Brian) and Tracy Williams (Jim); his siblings, Tony Butkus (Carol) and Margaret Check (Robert); his brother-in-law Jack Montgomery (Diana); his mother-in-law Ruth Montgomery (Richard); his close friend Bob Weronke; his grandchildren, Olivia of Joey, AJ, Roman and Tristan; and his nieces and nephews, Duane, Debbie, Jenny, Pam, Brian, Michelle, Anne, Julie and Rachelle.
