He was predeceased by: his parents, Frank Kotz and Josephine Kotz; and his stepfather Hank Ziarek. He is survived by: his spouse Robin; his daughter Hannah; his siblings, Mary Urlakis (Rick), William Kotz and Jan Kolb (Greg); his mother-in-law Gayle Lewis (late Frank); and his brothers-in-law, Randy and Greg Lewis (Laurie). He is also survived by nieces, nephews, great nieces and nephews as well as other relatives and friends.
