She was predeceased by: her parents, Jack Sullivan and Joanne Sullivan; her siblings, Mary Poker and Michael Sullivan; and her niece Mary Sullivan.

She is survived by: her sons, Darrin Leonhart (Gina) and David Jr. Eischen (Amanda); her grandchildren, Kayla, Olivia, Samuel and Lucas; and her siblings, John Sullivan (late Lynn), Rita Tischer (Steve), Pat Sullivan (Sandy), Mark "Bo" Sullivan (Lynne), Robert Sullivan and James "Chip" Sullivan (Julie). She is also survived by nieces, nephews, other relatives and friends.

In lieu of flowers, memorials to St. Jude Children's Hospital appreciated.
