He was predeceased by: his parents, Benito Baltazar and Frances Baltazar (Harman); his wife Diane T. Droskoski; and his brother Ben Baltazar. He is survived by: his sons, Ryan Baltazar (Becky) of Pleasant Prairie and Alan Baltazar (Katy Long); his grandchildren, Trent, Trace and Logan Baltazar; his brother Ron Baltazar of Chicago; and his sisters-in-law, Donna Baltazar of Chicago and Alice Kramer.

In lieu of flowers, memorials to the American Kidney Fund would be appreciated by the family.
