He was predeceased by: his parents, John Horak and Barbara Horak; and his brother Bob Horak. He is survived by: his children, Ryan Horak (Missie), Curtis Hinca (Brittany), Tim Millikin (Michelle), Tom Horak and Tammy Horak (John); his grandchildren, Lydia Horak and Julian Millikin; his sisters, Joan Toro (Scott), Judy Berg and Becky Scoles (Todd); and his nieces and nephews, Scott Toro, Jr. (Jaimie), Angela Toro, Dan Berg, Courtney and Keegan Scoles. He is also survived by other relatives, and dear friends.
