He was predeceased by: his parents, William Swart and Elizabeth Swart (Dekker); and his brother Robert Swart. He is survived by: his wife Carla J. Huibregtse; his sons, Dr. Daniel Swart of Mankato, MN, Dr. William Swart (Dawn) of Sioux Falls, SD and Dr. Gary Swart (Kerrie) of Milwaukee, WI; his grandchild Mackenzie Swart of Milwaukee, WI; his sons, Riley Swart of New York, NY, Liam Swart of Sioux Falls, SD and Kieran Swart of Sioux Falls, SD; his siblings, Miriam Heideman (Ralph) of Hingham, Bonita Peter (John) of Brandon, FL, David Swart (Barbara) of Rochester, MN and Gerald Swart (Susan) of Sheboygan Falls; and his sister-in-law Joan Swart of Hingham.
In keeping with Phillip's giving spirit, in lieu of flowers, memorials are suggested to the Hope Reformed Church benevolent fund.
