He was predeceased by: his parents, Fredric Smith and Helen Smith (Demling); his wife Cheri (Block); and his siblings, Fredric Smith and Virginia Patten.

He is survived by: his daughters, Elizabeth Warner (Chad) and Anne-Louise Plesh (Jason); his grandchildren, Nathan, Maggie, Sam and Lucas; and his nephew Fred Smith (Ella). He is also survived by extended family; and many dear friends.

In lieu of flowers, memorial donations may be made in David's name to Sharon S. Richardson Community Hospice or the Sheboygan County Food Bank.
