He was predeceased by: his grandparents, Ralph, Audrey Plass (TenPas), Edward and Hilda Osuch (Alwerth); and his uncles, Mark and Don Plass.
He is survived by: his parents, Lee Osuch and Debbie Osuch (Plass); his brother Logan Osuch (Sarah Schwefel); his niece Natalie; his nephew Oliver; and his sister Lindsay Streets (Tim). He is also survived by aunts, uncles, and cousins; and other relatives and friends.
In lieu of flowers, a memorial fund will be established in Adam's name.
