She was predeceased by: her parents, Earl Hazelwood and Marjorie Hazelwood (Hill); and her brothers, Don and Bill. She is survived by: her husband Roy Habeck of Shawano; her children, Judge James R. Habeck (Penny) and Jean Habeck; her siblings, Ann Lydon and Philip Hazelwood; and her siblings-in-law, Agnes Smolarek, Milton Habeck, Harold Habeck, Ralph Habeck and Ruben Habeck. She is also survived by many nieces, nephews, and friends.

Due to COVID-19 regulations, a celebration of Phyllis' life will be held at a later date.
