He was predeceased by: his parents, Charles Wazlawik and Mildred Wazlawik. He is survived by: his wife Elizabeth Wazlawik; his children, Wayne A. Wazlawik (Tina) of Topeka, Kansas, Marya Welch (James Welch) of Liberty, Missouri and Peter Wazlawik (Jenn Buntin) of Milan, Tennessee.; his grandchildren, Avery Welch, Ashlyn Welch, Sierra Wazlawik and Aiden Wazlawik; and his siblings, Charlene Faymoville (Duane), David Wazlawik (Donna), Jaclyn Wazlawik (Kevin Krafty) and Deborah Williams (Jon). He is also survived by numerous nieces and nephews.
In lieu of flowers, the family requests donations be made to the Veterans Assistance League in Cameron, Missouri.
