He is survived by: his wife Jennifer; his parents, Peggy Dvorak (Kent) and Allen Edward, Sr. Stevens (Leann); his siblings, Kristin Van Buren (Josh), Matthew Dvorak, Allyson Young (Kyle) and Kyle Dvorak; his nieces and nephews, Austin, Caleb, Ryan, Jayden and Bryce; his grandmother Patricia Von Asten; and also Dixie Rae. He is also survived by many aunts, uncles, and cousins.
