He is survived by: his parents, Paul Melotik and Wendy Melotik of Grafton; his siblings, Luke Melotik (Olivia), Ryan and Alicia Melotik; his grandfather Roger Thibodeau (Marge); and his aunts and uncles, Sue Tachick (Brian), Sandy Sievert (Randy), Sharon Dana (Andy), Renee Thibodeau (Clayton McIntyre), Daniel Melotik (Tiffany Monday), Marie Stelloh, Jerome Melotik and Michael Melotik (Doris). He is also survived by cousins, other relatives and friends.
