He was predeceased by: his parents, Stephen Wierzba and Lucille Wierzba (Jelinski); his in-laws, Johanna and Ambrose Kozikowski; and his brothers, Albert and Robert.
He is survived by: his daughters, Rebecca Lepak (David), Roxanne Filtz (Corky) and Sara Basinski (Kurt); his wife Alice Kozikowski; his great grandchildren, Scott Bembenek (Andrea), Alina, Cassie Borski (Dan), Elenie, Mavrie, Boone, Clayton Filtz, Zachary Lepak and Alex Basinski; his brothers, Bill, Larry, Jack (Marilyn), Don (Arlene) and Mike Wierzba; and his in-laws, Agnes Milanowski, Gladys Kluck, Norbert Kozikowski (Judy), Gayle Wierzba and Charlotte Wierzba. He is also survived by his three daughters and sons-in-law.
