He was predeceased by: his parents, Gerald Collins and Ruth Collins (Cooper); his brother Leo Collins; and his brother-in-law Mike O'Leary. He is survived by: his siblings, Maria T. Collins of Phoenix, AZ, Steve Collins (Kate) of Baraboo, Anne O'Leary of Richland Center, Joan Collins (Shannon Humphrey) of Kansas City, Maureen Collins of Richland Center and John Collins (Renette) of Richland Center. He is also survived by many nieces, nephews, other relatives and friends.
