He was predeceased by: his parents, Ward Booth and Edna Booth (Chapel); and his siblings, Evelyn Hanson (Harold), Peggy Schmidt (Ernest), Norma Richards (Nyrval) and Ward Booth (Betty).
He is survived by: his wife Sally Ann Port; his children, Susan and Gregg (Sherill); his grandchildren, Nathan and Blair; his great grandchild Sue Grunder (John); and also, Sammi Caulum (Tyler) and Emily. He is also survived by several nieces and nephews.
In lieu of flowers, memorials may be made to the United Methodist Church of Portage, the Alzheimer's Association, or Agrace Hospice in Richard's name.
