He was predeceased by: his parents, Gordon Wormer and Marjorie Van Wormer (Kerwin); and his brother Gary Van Wormer. He is survived by: his wife Sally Van Wormer (Beahm) of Portage; his sons, Michael Van Wormer (Stacy) of Pardeeville, Gerald Van Wormer of Portage and Jeffrey Van Wormer (Arin) of Fall Creek; his sisters, Sarah Manicki (James) of Almond and Jennie Johnson (Mike) of Montello; and his sister-in-law Judy Van Wormer of Baraboo. He is also survived by nieces, nephews, and other relatives.
