He was predeceased by: his father Dariel Cook; and his grandparents, Floyd, LaVina Farnum, Forrest and Martha Cook.

He is survived by: his mother Mary Cook (Farnum); his children, Jessica Orr (Justin), Kalie Cook and Cadin Cook; his grandchildren, Lillie Orr, Violet Kwallek and Ivy Fitzsimons; his sister Penny Sherwin; his niece Chelsey Sherwin (Shaun Juedes); and his great-nephew Mason. He is also survived by many aunts, uncles and cousins.
