He is survived by: his wife Mary Polacek; his daughters, Susan Hawkins (Greg) of Hammond, WI and Peggy-Sazzy Calhoun (Oskar Granquist); his grandchildren, Alex, Jill, Hayley and Luke; his great grandchildren, Colin, Natalie and Brooke; his nephews, Ralph and Andy Jr; and his niece Delores.
