We are sad to announce that on November 8, 2023, at the age of 88, Bernice H Polacek (Phillips, Wisconsin) passed away. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.

She was predeceased by: her parents, Edward Vokoun and Bessie Vokoun (Kavina); and her brothers, Edward and Jerome Vokoun. She is survived by: her husband Joseph Martin Polacek; her children, Mark Polacek (Dawn), Brian Polacek (Patrice), Jerel Polacek (Anita) and Janine Dobson (James); and her brother Raymond Vokoun. She is also survived by 10 grandchildren and 18 great-grandchildren.
