He was predeceased by: his parents, George J. Mazzuca and Dorothy E. Raska-Trapp; and his brother Ken. He is survived by: his children, Joseph G. (Kris), John D., James M. (Kris) and Tamara L. Mazzuca (Brent); and his grandchildren, Jacob, Brandon, Brady, Kailey, Brooklyn and Jackson. He is also survived by other relatives and friends.
In lieu of flowers, a memorial will be established in Jack's name.
