He was predeceased by: his parents, Marie Kouba and Jim Kouba; his stepfather Rudy Verbsky; and his siblings-in-law, Virgil, Lucille Myers and Judy Kouba.
He is survived by: his wife Linda Myers; his children, Jamie (Amber), Jason (Chris) and Joel Kouba (Rachel); his grandchildren, Axtyn, Andyn, Sydney Kouba, Bailey Joseph (Mitch), Jayce and Jayden Gorski; his siblings, Mary Baumgarten (Tom) and Bill Kouba; his aunt Angeline Boggiano; and his extended Jellystone. He is also survived by special friends the Midtlien family; many nieces and nephews.
In lieu of flowers, the family requests that donations be made to the American Diabetes Association or the American Heart Association.
