Puckett of Omro, WI.; his siblings, Madysen Puckett, Brianna Puckett and Brayden Puckett of Omro; his grandparents, Dennis, Karen Puckett (Welk) of Wautoma, WI, Dawn Foreman (Carl) of Omro, WI and Richard Munns; and his stepgrandmother Karen Munns (Andersen) of Racine, WI. He is also survived by numerous aunts, uncles, cousins and other family and friends.
