He was predeceased by: his parents, Richard Kratochvil and Laverne Kratochvil (Dallman); his siblings, Sue Kratochvil, Sandy Prochniak and Timothy Kratochvil; and his siblings-in-law, Dave Prochniak, Dennis Sadlon and Dorothy Jones-Spittlemeister.
He is survived by: his wife Nancy L. Hartman; his stepson Aaron Lohman (Stephanie Pauls); his sisters, Donna Sadlon, Sherry Warden and Lesa Patt (Kane); his parents-in-law, Lawrence and Marlene Hartman; and his siblings-in-law, Jeffrey Hartman (Debbie), Kelly Vinge and Marie Paszkiewicz. He is also survived by nieces, nephews, other relatives and many friends.
Memorial contributions can be made in David's name and directed to the family for later distribution.
