He was predeceased by: his parents, Leonard Polaske and Mary Polaske (Rohan); his brothers, Clement and Leon; his niece Mary Ellen Mann; and his nephews, Michael Mann and Matthew Mann.

He is survived by: his niece Debbie Guckert; his nephews, Neil Polaske, Mark Mann and Christopher Mann; his friends, Dave Neumann and Jodi Brennan; and also Katherine.
