He was predeceased by: his parents, David R. Lewis and Lelah Lewis (Chase); his sister Pamala Moss; and his grandson Owen Lewis. He is survived by: his wife Patricia M. Maki of New Lisbon; his children, Terry Lewis (Katie) of New Lisbon, Christine Lewis of Paola, KS, Susan Shindler (Francis) of Kendall, WI, David R. Lewis (Sherri Matusik) of Baraboo, WI and Troy Lewis of New Lisbon; and his siblings, Calvin Lewis (Anne), Jon Lewis of West Salem, WI and Heather Lannon (Kevin) of Colfax, WI. He is also survived by 13 grandchildren and 9 great grandchildren.
