He is survived by: his wife Susan Kay Kaminski (Lemon); his daughters, Linda Trewyn (Jonathan) and Karen Early; his grandchildren, Collin, Evan, Ellise, Jamison Trewyn, Kylie Early and Keegan Early; his sister-in-law Sharon Kaminski; his brother Alan; and his parents, Regina D. and Max B. Kaminski.

In lieu of flowers, memorials are appreciated to: CHILDREN'S HOSPITAL OF WISCONSIN or WISCONSIN HUMANE SOCIETY.
