He was predeceased by: his parents, Terry Bellak Jami and Don Bellak; his grandparents, Reba, Jack Bellak of Philadelphia, PA, Viola and Michael Ebner of Laredo, TX; and his stepmother Pamela Bellak (Don).
He is survived by: his significant other Dax; his sister Kimberly Kristy Gaskill; his brother-in-law Dave; his nephews, Ryan Rosborough (David), James "Beau" June and Josh Gaskill of Las Vegas; his niece Alexandra June; his brother Jason; his sister-in-law Jennifer; his niece Isabelle; his nephews, Hunter and Tyson of Edmond, OK; his stepsister Bridgette Gardener; his uncle Michael Ebner; his great-nieces and great-nephews, Londyn, Avery, Emery, Eagan and Pierce; and also, Alyssa Hill, Drew Gutzwa of Las Vegas and Evelyn Nessman (Henry).
