He was predeceased by: his parents, Marian Smith (Laurin) and Douglas Smith; his siblings, Donald Smith (Bernice), DeVon Smith (Audrey), Garland Smith (Edna), Marvel Buksyk (David) and Beverly Smith; his stepdaughters, Phyllis Mande and Gloria Du Fay; his in-law Richard Moran (Lieselotte); his stepmother Viriginia "Ginny" Moran; and his brother-in-law Tommy Moran.

He is survived by: his wife Veronica "Bim" Smith (Moran); his sons, Rick Smith (Amy Erickson) and Michael Smith (Cole Heeren); his sister Donna Kopitzke (Harley); his siblings-in-law, Rick Moran, Mike Moran (Mary), Patti Van Dinter (James), Christa Bednarowski (Dave) and Lisa Moran; his grandchildren, Amanda Ringer (Kevin), Rickey Smith, Teri Smith and Navaeh Smith; and his stepgrandchildren, Glenn, Tina, Lee Ann (Sean), John, Richard, Crystal (Jeremiah), Austin and MyKenzie. He is also survived by numerous great grandchildren, nieces, and nephews.

In lieu of flowers, a memorial fund honoring Perry is being established.
