He was predeceased by: his parents, Paul Herbert and Margaret Herbert (Grover); his siblings, Adam and Peggy; his mother-in-law Jan; his aunt Pat; and his cousins, Joey, Mary Agnes and Patti Ann.
He is survived by: his wife Jennifer; his children, Michael Corey (Tawny Knutson) and Megan O'Loughlin; his close friend Charles; and his siblings, Paul O'Loughlin (Maggie), Todd Kolk, Renee Moore (Ivan), Danny O'Loughlin (Anna), Mikey O'Loughlin, Johnny O'Loughlin (Cassie), Joey King (Kevin), Mary O'Loughlin (Greg Gavin) and Jessica Lamacki (Matthew). He is also survived by many nieces, nephews, aunts, uncles, cousins and friends.
