He was predeceased by: his parents, Lyle II and Audree Perkins (Dowling); his wife Penny; and his son Lyle Perkins IV. He is survived by: his siblings, Patricia Allen of Menasha, Lynn Burton of Appleton, Loreen Perkins-Pozolinski (Ron) of Menasha, Lonny Perkins (Debby) of Larsen and Lisa Sample (Bob) of Mountain; and his friend Sue Gering. He is also survived by niece, many nephews, other relatives and friends.

In lieu of flowers, a memorial fund has been established.
