She was predeceased by: her parents, Clayton Campbell and Deloris Campbell (Paulson). She is survived by: her siblings, Judy Weiss (Joe) of Montello, Richard Campbell (Joanie) of Baraboo, John Campbell (Sherri) of Montello and Dan Campbell (Diana) of Montello; and her cousin Doug Paulson. She is also survived by many nieces, nephews and other extended family.
In lieu of flowers, donations are preferred to the family for a memorial that will be established in Phyllis's name.
