He was predeceased by: his parents, Myron Bolks and Juella Bolks; and his sister Betty Bolks. He is survived by: his wife Susan; and his daughters, Heather Evenhouse (Charlie) and Cathy Muir (Bill). He is also survived by nine grandchildren.
