He was predeceased by: his parents, Fay Cushman and Mary Cushman (Goninen); his former wife Beverly Bollerud; his son Kelly; and his brothers, Clarion Cushman and Dwayne Cushman.
He is survived by: his wife Bev; his niece Ann Dorner of Linden; and his cousin Gary Cushman (Beverly) of Mount Horeb. He is also survived by several nieces, nephews, relatives and friends.
