He was predeceased by: his parents, Jean Lewis (Stehr) and John J. Lewis. He is survived by: his wife Rhonda (Kurth); his daughter Amber; his brother Mark; his siblings-in-law, Linda, Tom, Terry, Kathy, Brian and Wade; his friend Jim (John); and also, Donna, Lisa, Pam, John, Rhonda and Amber. He is also survived by nieces, nephews, great-nieces and nephews, and other relatives and friends.
