He is survived by: his wife Nancy Mayer; his children, Greg Mayer (Jamie), Jeff Mayer (Vicki), Jody Anklam, Jamie Mayer (Heidi) and J. P. Mayer (Niki); his grandchildren, Kayleigh (Trevor), Kyle (Danielle), Aaron, Janey, Kylie, Nadine, Carson, Jackson, Alissa and Talie; his siblings, Shirley Slomski and Beverly Lobermier (Craig); and his siblings-in-law, Norbert Berghuis, Joyce Meyer and Marilyn Miller (Mark). He is also survived by many nieces, nephews, relatives and friends.
In Lieu of flowers, a memorial will be established in Jack's name.
