He was predeceased by: his parents, Rufus Slaby and Bernice Slaby (Haines); and his brothers, Ronald and Daniel Slaby. He is survived by: his wife Jacqueline Francis of Anthony's Catholic Church; his children, Todd Slaby (Katie) of Chetek, Kelly Mathis (Johnny) of Marshfield and Jason Slaby (Jen) of Marshfield; his grandchildren, Makayla, Brianna, Olive, Nathan and Alissa; his great grandchild Davina; and his siblings, Doris, Dale and Diane.
