She was predeceased by: her parents, John Chlebowski and Margaret Chlebowski (Zinda); and her husband Anthony "Dean" Hoffman. She is survived by: her former husband Gary Zawacki; her sons, Gary Zawacki (Patty), Shawn Zawacki (Ann) and Jesse Hoffman (Krystal); her grandchildren, Akiya, Luikai, Maxwell and Eugene; and her siblings, Patricia Bartelson (Jerry), Kenneth Chlewbowski and Shirley Ann Kier. She is also survived by nieces, nephews, other relatives and friends.
