He was predeceased by: his father Ron Kooistra. He is survived by: his wife Carrie; his children, Bretton Kooistra (Lauren), Connor Kooistra, Lilly Kooistra and Annabel Kooistra; his mother Marsha Kooistra; his parents-in-law, Don and Karen Marema; his siblings, Michael Kooistra, Sarah Groves (Pat) and Dave Kooistra (Nichole); and his sisters-in-law, Melissa Nelson (Eric) and Katie VanHaven. He is also survived by 9 nieces and nephews and many special friends.
