We are sad to announce that on November 5, 2018, at the age of 77, Phyllis M. Sillman (Lake Mills, Wisconsin) passed away. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.

She is survived by: her parents, Anna Frances (Huber) and Bernard Kasuboske; her husband Philip; her brother James (Jane); her nephew Matthew; and her niece April.

Memorial donations may be given to the Lake Mills EMS, Waterloo EMS, and L.D. Fargo Library in Lake Mills.
